Health Information

When you create an account with EvermoreMD, you establish a direct relationship with EvermoreMD for purposes such as account administration, billing, payment processing, customer support, communications, platform access, and related operational services.

In connection with these Services, EvermoreMD may collect personal information such as your name, email address, phone number, shipping address, payment information, date of birth, device information, and transactional information.

Certain information collected solely for account administration, customer support, payment processing, marketing, analytics, or platform operations may not constitute protected health information ("PHI") under HIPAA when maintained independently from medical records maintained by licensed healthcare providers.

In connection with healthcare-related services facilitated through the Services, you and applicable Healthcare Providers may exchange health, medical, prescription, pharmacy, laboratory, wellness, or treatment-related information that may constitute protected health information or other protected data under HIPAA or applicable state privacy laws.

EvermoreMD works with independent licensed Healthcare Providers, pharmacies, laboratories, and other healthcare organizations to facilitate healthcare-related services requested by users. These entities are independently owned and operated and are responsible for their own professional, regulatory, privacy, pharmacy, prescribing, dispensing, and healthcare compliance obligations.

Healthcare Providers and pharmacies may maintain separate records, privacy practices, and legal obligations governing the healthcare services they provide to you.

Consumer Health Data

Certain information collected through the Services may constitute "consumer health data" or other sensitive personal information under applicable state privacy laws, even if such information is not protected health information under HIPAA.

Such information may include information relating to your health conditions, symptoms, treatment interests, prescription interests, wellness information, healthcare interactions, or other health-related data.

We collect, use, disclose, and protect such information only as reasonably necessary to provide the Services, facilitate healthcare-related services requested by you, comply with legal obligations, protect against fraud and misuse, improve our Services, or as otherwise permitted by applicable law.

The Services may only be used within certain states within the United States as described in our Terms and Conditions. Accordingly, this Privacy Policy, and our collection, use, and disclosure of information about you, is governed by U.S. law.

Role of EvermoreMD

EvermoreMD provides administrative, technology, customer support, billing, marketing, and platform-related services that facilitate access to healthcare services offered by independent licensed medical providers, pharmacies, laboratories, and other healthcare entities.

EvermoreMD does not itself practice medicine, provide medical advice, prescribe medications, dispense medications, operate a pharmacy, or provide healthcare services. Medical decisions, treatment determinations, prescriptions, and clinical services are provided solely by independent licensed healthcare professionals and healthcare entities.

Any healthcare services you receive through the Services are provided by independent Healthcare Providers, which maintain responsibility for compliance with applicable medical, pharmacy, privacy, and professional standards.

Changes To This Privacy Policy

This Privacy Policy is effective as of 5/7/2026 and will remain in effect except with respect to any changes in its provisions in the future, which will be in effect immediately after being posted on this page.

We reserve the right to update or change our Privacy Policy at any time and you should check this Privacy Policy periodically. Your continued use of the Service after we post any modifications to the Privacy Policy on this page will constitute your acknowledgment of the modifications and your consent to abide and be bound by the modified Privacy Policy.
